Signed Application
If any corrections need to be made, please have the insured make those corrections on the application.
Central Station Fire/Burglar Alarm Certificate
If installed, we will need a copy of the alarm certificate for the company in order to keep alarm discount on the policy. Without the proper certificate, the company will remove the discount.
Current Homeowner/Dwelling Fire Policy
Unless this is a new purchase, we need a copy of the insured’s current homeowners or dwelling fire policy for the insured location in order to show there has been no lapse. Proof of coverage must show, the named insured, the insured property and the effective dates.
Mortgagee Clause & Loan #
If a mortgage company is to be listed on the policy, please provide the mortgagee clause and loan # at the time of binding.
Copy of Current Appraisals
If there is any scheduled personal property listed on the quote, we will need a recent appraisal at the time of binding.
Completed Payment Form
We need either payment in full or a down payment for an installment payment plan at the time of binding. Please be advised that all 12-pay options must be setup using checking account information. None of our companies will do a recurring credit card. If the policy is to be billed to the mortgage company, please indicate that in your bind request email.
